IMPORTANT NOTES
Cats need to scratch. If you need information about scratching posts or trimming your new pet's nails, please discuss this with one of the shelter's volunteers. We are eager to assist however we can so that both you and your new pet will be happy.
Pets need veterinary care throughout their lives. Before deciding to adopt a pet, please be sure that you've budgeted not only for food, but also for annual vaccines and check ups. Please do not adopt a pet if you will not be able to provide it with ongoing care.
The Medfield Animal Shelter is committed to doing our part to help end the problem of cat overpopulation. To that end, every pet adopted from the Medfield Animal Shelter will be spayed or neutered. Due to their young age, certain kittens may go to their new home prior to being altered. In this event, I agree to bring my pet(s) back to the shelter for his or her surgery as scheduled by the Medfield Animal Shelter.
I certify that the information I have given is true, and I understand the Medfield Animal Shelter may contact my veterinarian, personal references and landlord to verify the information on this form. I also agree that if the adoption doesn't work out for any reason, I will return the pet to the Medfield Animal Shelter.
